This comprehensive guide will walk you through the process of signing up for our platform, ensuring a seamless and hassle-free experience.

Follow these steps to get started on your healthcare journey.

Step 1: Open the CareMonitor Patient App

Unlock your mobile device and locate the CareMonitor Patient App icon.
Tap on the app icon to launch the CareMonitor Patient App.

Step 2: Tap on Signup

Once the app is open, you'll be directed to the welcome screen.
On the welcome screen, locate and tap on the "Signup" option to begin the signup process.

Step 3: Enter Your Details

On the signup screen, start by entering your last name into the designated field.
Once you've entered your last name, tap on the "Next" or "Continue" button to proceed to the next step.
You'll then be prompted to enter your date of birth. Input your date of birth into the provided field.

After entering your date of birth, tap on the "Next" or "Continue" button to proceed further.
Lastly, you'll need to enter your mobile number into the designated field.

Once you've entered your mobile number, tap on the "Get Code" button to request an OTP (One-Time Password) for verification.

Step 4: Verification

If your details match our records:

- We will send you an OTP (One-Time Password) via SMS to your provided mobile number.
- Retrieve the OTP from your SMS inbox and enter it into the designated field within the app to verify your identity.

If you don't receive the OTP within a few minutes, you can tap on the "Resend" button to request

the OTP again.

Step 5: Review Your Email and Set Password

After successful verification, you will be prompted to review or set up your email address.
Confirm your email address to proceed.
Set up a password for your account. Choose a secure password that you can easily remember.

Step 6: Accessing the Home Page

Once your account setup is complete, and after agreeing to the consents and Terms and Conditions, you'll be automatically directed to the home screen of the CareMonitor Patient App.
Explore the various features available to you, including health data tracking, appointment scheduling, and more.
